EARLY INTERVENTION CASE MANAGER ­ POSITION #YE-620-4 ­ Provide assessment, linkage, and advocacy services to infants & toddlers under the age of 3 & their families in Martinsville, Franklin, Henry, & Patrick Counties. Acts as primary facilitator in the coordination & integration of early intervention services. Bachelor's degree in early childhood education, special education, human services related field, or any discipline or profession listed under Part C Personnel Standards; or a Registered Nurse with a valid license to practice in Virginia; or any equivalent combination of education, training, & experience, which provides the required knowledge, skills, & abilities. Prefer that Early Intervention Professional Practitioner Level be met for a discipline or profession as defined in Chapter 12 of the Infant Toddler Connection of Virginia's practice manual. Prefer at least 1 year of experience working with children under the age of 6 with developmental disabilities or developmental delays, case management, and writing/implementing multidisciplinary service plans. Requires excellent communication skills and experience in working pleasantly and cooperatively with a wide variety of people.
